Kan Alma ve Kan Ürünlerine Dönüştürmenin Birim Maliyet Analizi ve Sağlık Uygulama Tebliği Fiyatları ile Karşılaştırılması

Abstract
Araştırmanın amacı; bir Üniversite Hastanesi’nde bağışçılardan kan alma ve kan ürünlerine dönüştürme maliyetinin hesaplanması ve Sağlık Uygulama Tebliği (SUT) fiyatları ile karşılaştırılmasıdır. Çalışma retrospektif türde olup 01 Ocak- 31 Aralık 2021 tarihlerini kapsamaktadır. İlk önce bağışçıdan kan alınması ve kan ürünlerine dönüştürülmesi sürecinin adımları belirlenmiştir. Daha sonra 2021 yılında oluşan tüm giderler ile üretilen kan bileşeni sayıları belirlenmiştir. Maliyet hesaplamasında; işçilik ve malzeme maliyetlerinin mikro maliyetle, genel üretim maliyetlerinin ise brüt maliyet yöntemiyle hesaplandığı hibrit bir model kullanılmıştır. Araştırma sonucunda; Eritrosit Süspansüyonu birim maliyeti 430,36 TL (48,52 $), Trombosit Süspansüyonu birim maliyeti 294,96 TL (33,25 $) ve Taze Donmuş Plazma birim maliyeti 348,69 TL (39,31 $) olarak bulunmuştur. Bu maliyetler Sosyal Güvenlik Kurumunun hastanelere yaptığı ödeme rakamlarının üstündedir. Geri ödeme kurumlarının bu tarz maliyet analizi hesaplamalarını ve sonuçlarını dikkate almasında yarar bulunmaktadır.

Abstract
The aim of the study is to calculate the cost of blood collection from donors and conversion into blood products in a University Hospital and to compare it with the Health Implementation Communiqué (HIC) prices. The study is retrospective and covers the period between January 1 and December 31, 2021. First, the steps in the process of taking blood from the donor and converting it into blood products have been determined. Then, all expenses incurred in 2021 and the number of blood components produced were determined. In the cost calculation, a hybrid model was used in which labor and material costs were calculated by micro costing and overhead costs were calculated by gross cost method. As a result of the research, the unit cost of Erythrocyte Suspension was 430.36₺ ($48.52), Platelet Suspension unit cost was 294.96₺ ($33.25) and Fresh Frozen Plasma unit cost was 348.69₺ ($39.31). These costs are higher than the payments made by the Social Security Institution to hospitals. It would be useful for reimbursement agencies to consider such cost analysis calculations and results.
